Lookman handed Atletico the lead in the 33rd minute, showing composure to bring down a clever flick from Julián Álvarez before finishing neatly past the goalkeeper.
The goal made him the first Nigerian to score in the historic fixture.
Atletico held their advantage going into halftime, but the momentum shifted quickly after the restart.
Vinícius Júnior drew Real level in the 52nd minute from the penalty spot after Brahim Díaz was brought down inside the area.
Moments later, Federico Valverde completed the turnaround with a precise low drive to put the hosts in front.
Atletico responded in the 66th minute through Nahuel Molina, who rifled home a stunning long-range effort to make it 2-2 and set up a tense finale.
However, Vinícius proved decisive once again, curling home a fine strike in the 72nd minute to restore Real’s lead and ultimately secure all three points.
The closing stages added further drama when Valverde was sent off in the 77th minute, but goalkeeper Andriy Lunin delivered a string of crucial saves to deny Atletico and seal the win.
The result strengthens Real Madrid’s push in the title race, reducing the gap to leaders FC Barcelona to four points, while Atletico remain fourth in the standings.
